A one-page sheet for the gate: date, name, company, mobile, what they're here for, whether they've been inducted, white card number, time in, time out and a signature. Sixteen rows, landscape A4, so it fits a clipboard and a sheet lasts most of a busy day.

What a sign-in sheet is for
A sign-in sheet answers two questions. Who is on site right now? And who was here on a given day?
The first matters most in an emergency. If you have to clear the site, the sheet is how you check everyone got out. That only works if people sign out as well as in, which is why the time-out column is as wide as the time-in one.
The second matters weeks later, when a client queries a day, a head contractor asks who was working near the scaffold, or you are piecing together an incident. A dated sheet with names and companies is the record.
What goes in each column
Date. On every line. Sheets get stapled together and pages go missing, and each line should still say which day it was.
Name and company. Full name, and the business they work for. "Dave, plumber" helps nobody in six months.
Mobile. So you can find them if they wander off site without signing out.
Here for. One or two words: framing, delivery, inspection, quote. It tells you whether a person needed an induction at all.
Inducted? A tick in the Y/N column. Anyone answering N is the person to stop at the gate and walk through the site rules before they start. A sign-in sheet records attendance. The induction is a separate step, and it happens once per person per site.
White card number. Construction workers need a current general construction induction card. Writing the number down is how you show you checked it.
Time in, time out, signature. The signature is what turns a list of names into a record someone vouched for.
What people get wrong
The sheet lives in the ute. If it isn't at the gate, nobody signs it. Clip it to something fixed, with a pen on a string.
Nobody signs out. Most sheets have a column of times in and a column of blanks. Put the sheet where people walk past it when they leave, and make signing out the habit at knock-off.
Visitors skip it. The client, the engineer and the council inspector all count. If they come past the gate, they sign.
Old sheets go in the bin. Keep completed sheets with the job's site records, alongside the site diary and the induction records.
Doing it without the clipboard
Foreman's QR site inductions take the induction part off paper. Print the QR poster for the gate. Anyone who scans it reads your site documents on their own phone, fills in their details and white card number, and signs. There is nothing to install and no login, and the office can check each site's induction register from anywhere.
For your own crew, Foreman can also run check in and check out. Crew scan the same code when they arrive and tap Check out when they leave, and each shift saves to the site diary with the time on site. The On site now card shows who is checked in at the moment. Check-in is for crew with Foreman logins, so visitors and subbies who only induct are never billed, and they don't appear in that list. Keep this sheet at the gate for daily visitor sign-ins.
You can run all of it on your first project for free, with every feature and unlimited users on it.
