How to build a form template in Foreman
Build a form once and your crew fill it in on their phone on every project. Toolbox talks, incident reports, checklists and permits all start here.
In Foreman, an Admin builds each form once as a template: an incident report, a toolbox talk, a pre-start checklist, a permit. The template then appears in the Forms list of everyone it's meant for, on every project they work on. Crew fill it in on their phone, with photos and a signature if you've added them, and the submission is saved against the job.
Only an Admin can build or change form templates. Admins, Site Managers and Site Crew fill them in, depending on who each form is for.
Toolbox talks are a template you build
Foreman doesn't ship with a library of ready-made safety forms. A new account starts with one sample template, which you can edit or archive like any other. Everything else, toolbox talks included, you build to match how you already run things on site. It takes a few minutes, and you end up with a form that asks the questions you'd ask anyway, instead of someone else's generic list.
Build a template
- Open Forms and tap Manage forms. On a phone, Forms is under More.
- Tap + New form.
- Give it a Form name, such as "Toolbox talk".
- Under Who can fill this, choose All site crew, or Site managers + admins only for forms like site inspections that only your leads should fill in.
- Under Form type, choose Project form if it belongs to a job (most do). Choose Other form for things that aren't about a site, such as a leave request.
- Tap + Add first block and pick a block. Keep adding blocks with + Add block until the form is complete.
- Tick Required on any input that must be filled in before the form can be sent. To-dos blocks don't have this option.
- Tap Preview to see it the way your crew will.
- Tap Save form. It's live straight away.
The blocks you can use
Content blocks show information but don't capture anything. Heading splits the form into sections. Paragraph holds instructions, such as the points to cover in today's talk. Divider draws a line.
Inputs capture answers:
- Text for a typed answer, like a description of what happened.
- To-dos for a checklist of items to tick. This is where checklists live in Foreman.
- Single select to pick one option from a list you write, and Multi select to tick as many as apply.
- Date for a date. Tick Lock to submission date and the field always shows the day the form is sent, so crew filling it in can't backdate it.
Advanced blocks:
- Photo opens the phone's camera. Photos go straight into the form and aren't saved to the phone's camera roll.
- Signature lets the person draw their signature on screen.
- Project asks which project the answer relates to.
Move blocks up or down with the arrows, and remove one with the cross.
An example: a toolbox talk
A toolbox talk template might be a Date block locked to the submission date, a Text block for the topic, a Paragraph with the points you want covered, a To-dos list of what was discussed, and a Signature at the end.
Plan for this one: a Signature block captures one signature. For attendance, either have each worker submit the form themselves, or add a Text block for the attendees' names and have the person running the talk sign.
How your crew fill it in
Site Crew open Forms, stay on Available to fill, and tap the form. Forms are grouped under Project forms and Other forms. For a project form they pick the project first. If they start from the Project forms card on a project page, the project is already chosen.
They fill in the fields and tap Submit. Submit stays greyed out until the required fields are done. A confirmation screen then shows the form was sent.
Forms need a connection to send, and there's no saving a half-finished form. If someone leaves the page partway through, they start again, so it's worth finishing it in one go.
Where submissions are stored
Every submission is saved with who sent it, when, and the project. You'll find them under Forms in the Submissions tab. Admins see every submission and can filter by form, project and submitter. Site Managers see the project form submissions on their sites. Site Crew see only their own, read-only. Submissions of Other forms, like leave requests, are only seen by the person who sent them and Admins. The project page also has a Form submissions card for Admins and that project's Site Managers.
A project's submissions are included in its project data export, as a CSV.
Good to know
- Editing a template doesn't change what's already been submitted. Each submission keeps the version of the form it was filled against, so go ahead and tidy a template up whenever you like.
- To retire a form, open it from Manage forms and choose Archive from the actions menu. It disappears from your crew's list and its submissions stay. Make active brings it back.
- A form can only be deleted if nobody has submitted it yet. Otherwise archive it.
- A template needs at least one input block. Headings and paragraphs alone can't be saved.
